For the first time in three years, fewer employers are paying for GLP-1 weight loss. A WTW survey released October 28, 2026 found that 53% of U.S. employers with 100 or more workers cover GLP-1 medications for weight loss, down from 57% a year earlier, and 30% of those still covering them are considering dropping it. A week earlier, KFF reported that only 18% of large firms cover these medications for weight loss at all. With open enrollment underway, here is what the numbers mean if you live in Portland or Vancouver and your 2027 plan is changing.
The short version
- WTW: employer coverage of GLP-1s for weight loss fell from 57% to 53%, the first decline in three years.
- Of employers still covering them, 30% are considering eliminating coverage, and 91% of those dropping or weighing a cut cite cost.
- KFF: 18% of firms with 200 or more workers cover GLP-1s for weight loss in their largest plan, from 16% of mid-size firms to 33% of the largest.
- Among firms that do cover them, 40% require a dietitian, case manager, therapist or lifestyle program as a condition of coverage.
What the two new surveys found
WTW’s 30th annual Best Practices in Healthcare Survey drew on 471 U.S. employers with at least 100 employees, together employing 7 million people, surveyed between June 22 and August 7, 2026. Pharmacy costs are expected to rise 11.5% in 2026, up from 9% in 2025, and GLP-1 coverage for weight loss declined for the first time in three years.

The KFF 2026 Employer Health Benefits Survey, published October 22 and based on 1,937 randomly selected employers, looked at the question differently: whether a firm’s largest health plan covers GLP-1 medications used primarily for weight loss. Coverage rises with company size.

Why the 53% and 18% figures are both right
Reading the two surveys side by side, it can look as if they contradict each other. They do not. WTW counts the share of employers with 100 or more workers that offer GLP-1 weight-loss coverage. KFF counts the share of firms with 200 or more workers whose single largest plan includes it, weighted across thousands of mid-size firms that are far less likely to cover these medications.
The useful takeaway for an employee is that coverage depends heavily on the size of your employer and the specific plan you choose. KFF found that 12% of large firms not covering GLP-1s for weight loss had stopped within the last 12 months, and 79% said they were not likely to start in the next year.

What employers are asking for when they do cover it
KFF reported that 40% of firms covering GLP-1s for weight loss, including 59% of firms with 5,000 or more workers, require enrollees to meet with a dietitian, case manager or therapist, or join a lifestyle program, before the medication is covered. That share was 34% a year earlier. CNBC reported that employer coverage typically costs in the mid-$500s to mid-$600s per user each month, which explains why plans are adding conditions.
The angle most coverage missed
Employers are quietly redefining what a weight-loss program has to include
The coverage cuts got the headlines. The bigger shift is in what the remaining coverage looks like. Four in ten covering employers now tie the medication to ongoing support from a dietitian, case manager, therapist or structured lifestyle program, and that share is climbing.
KFF’s own conclusion points the same way. It notes that lower prices available through direct-to-consumer arrangements, together with the cost of these medications, may lead employers to keep adapting coverage. In other words, more people will be comparing self-pay options, and the programs that hold up will be the ones that include monitoring, dose adjustments and follow-up rather than a prescription alone.
There is also a tax detail many people miss. CNBC notes that GLP-1 medications are eligible for FSA and HSA reimbursement when prescribed by a licensed provider to treat a diagnosed medical condition such as obesity. The 2026 FSA contribution limit is $3,400, and HSA limits rise to $4,500 for individuals and $9,000 for families in 2027. Your plan administrator can confirm what applies to you.
What this means in Portland and Vancouver
Open enrollment is the moment to read your 2027 plan documents closely, because a medication covered this year may not be next year. Use of these medications keeps climbing: according to Gallup, as reported by CNBC, 11% of U.S. adults were taking a GLP-1 for weight loss in 2026, up from 3% in 2024.
If your coverage is ending, a medically supervised program is one option to compare.
