Terms of Service for our website, bookings and treatments

These terms govern your use of cosmetiqmedicine.com and the services we provide at our Portland, Oregon and Vancouver, Washington clinics. Please read them before booking. By using this website or booking an appointment, you agree to them.

Article OneAgreement to these terms

These Terms of Service form an agreement between you and Cosmetiq Medicine LLC, an Oregon limited liability company operating clinics in Portland, Oregon and Vancouver, Washington. In these terms, “we,” “us” and “our” mean Cosmetiq Medicine LLC. “You” means the person using the website or receiving services.

By browsing this website, submitting a form, booking an appointment or receiving a treatment, you accept these terms. If you do not agree with them, please do not use the website or book an appointment.

These terms work alongside our privacy policy, which explains how we handle your information. Treatment-specific consent forms you sign at the clinic also apply, and where a signed consent form conflicts with these terms, the signed form controls for that treatment.

Who may use our services

  1. You must be at least 18 years old to book an appointment or agree to these terms on your own behalf.
  2. Minors may receive certain treatments only where clinically appropriate, with a parent or legal guardian present, and with that adult providing written consent and accepting financial responsibility.
  3. Website use is not directed to anyone under 13, and we do not knowingly collect information from anyone under 13.
  4. Candidacy is determined by a provider. Booking an appointment does not guarantee you are a candidate for the treatment you requested. We may decline to perform a treatment where it is not clinically appropriate.

The content on this website is general information, not medical advice. It is not a substitute for a consultation, diagnosis or treatment by a qualified provider, and it does not create a provider and patient relationship between you and Cosmetiq Medicine.

Never disregard professional medical advice, or delay seeking it, because of something you read here. If you think you may have a medical emergency, call 911 or your local emergency number.

Results vary

Aesthetic outcomes depend on your anatomy, skin quality, health history, age, the product or device used, how many sessions you complete and how you follow aftercare. Before and after photographs, testimonials and results described on this website reflect the experience of individual clients and are not a promise or guarantee of your result. No provider can guarantee a specific outcome.

Risks and informed consent

Every treatment carries risk. Before any treatment you will receive information about its risks, benefits, alternatives and expected recovery, and you will be asked to sign a consent form specific to that treatment. Ask questions until you are satisfied. You may decline or stop a treatment at any point.

Pricing and product information

We work to keep published prices and treatment descriptions accurate. Prices, product availability and promotions may change without notice, and typographical errors occur. The price quoted to you in writing at consultation, before treatment begins, is the price that applies. We reserve the right to correct an error in a published price and to decline a booking made at an incorrect price.

Article ThreeAppointments, cancellations and no-shows

Booking

Appointments may be booked online through our booking platform, by phone or in person. A booking is confirmed when you receive a confirmation from us. Some appointments require a deposit or a card on file to hold the time.

Cancellation and rescheduling

We ask for at least 24 hours' notice to cancel or reschedule an appointment. Notice given at least 24 hours in advance carries no charge, and any deposit is applied to your rescheduled visit or refunded at your request.

  1. Late cancellation. Cancelling with less than 24 hours' notice may result in forfeiture of your deposit or a cancellation fee charged to the card on file.
  2. No-show. Failing to attend without notice may result in forfeiture of your deposit or a no-show fee, and we may require prepayment for future bookings.
  3. Late arrival. Arriving late may shorten your treatment time or require rescheduling, because the appointment after yours belongs to someone else.
  4. Our cancellations. If we must cancel or reschedule, we will contact you as soon as possible and no fee applies. Any deposit is refunded or moved to your new appointment at your choice.
  5. Circumstances beyond control. We waive cancellation fees for genuine emergencies, illness and severe weather. Tell us what happened and we will work with you.

Arrival and conduct

Please arrive with the treatment area clean and free of makeup where we have asked for that, and tell us about any change to your health, medications or pregnancy status since your last visit. We may refuse or discontinue service to anyone who is abusive to staff or other clients, who appears impaired, or who withholds information that makes treatment unsafe.

Article FourPricing, payment and refunds

Payment

Payment is due at the time of service unless we have agreed otherwise in writing. We accept cash, credit and debit cards, CareCredit, Cherry payment plans, Allē rewards, gift cards and membership credit. You authorize us to charge the card you provide for services rendered and for any fee described in these terms.

Refunds

Because aesthetic treatments are performed on the body and cannot be returned, treatments already performed are not refundable. This includes cases where your result differs from what you hoped for, which is why we discuss realistic expectations before treatment.

  1. Unused prepaid services and packages may be refunded at our discretion, less the value of any service already used at its regular non-package price.
  2. Retail products may be returned unopened and unused within 14 days of purchase. Opened products cannot be returned for hygiene reasons.
  3. Deposits are applied to the cost of your treatment and are handled as described in Article Three.
  4. Touch-ups and adjustments, where offered, are handled as a clinical matter rather than a refund. Contact us within the window your provider gives you at treatment.

Third-party financing

CareCredit and Cherry are independent companies. When you apply for financing, you enter an agreement directly with that company under its own terms, and it decides whether to approve you. We do not control approval, rates or repayment terms, and we are not responsible for their decisions. Details of what we accept are on our payment plans page.

Gift cards

Gift cards are redeemable for services at either clinic. They are not redeemable for cash except where state law requires it, cannot be replaced if lost or stolen, and cannot be combined with certain promotions. Gift card balances do not expire in value.

Article FiveMemberships

Our Glow and Elite memberships are recurring monthly agreements with a minimum commitment. Full details are on our memberships page, and the terms below form part of your agreement when you enroll.

What it means
Monthly payment
Glow is $100 per month. Elite is $200 per month. Payment is charged automatically to the card on file on your enrollment date each month.
Minimum commitment
Glow requires six months. Elite requires twelve months. You remain responsible for payments through the end of your commitment term.
Banked credit
Each monthly payment is banked as credit toward treatments, dollar for dollar. Credit accumulates and may be used on any regular-priced service.
Member pricing
Glow members pay $11 per unit for Botox and Dysport and receive 10 percent off regular-priced services. Elite members pay $10 per unit and receive 15 percent off.
Loyalty balance
Your first monthly payment is held as a loyalty balance and returned to you as credit after you complete your commitment term.
Elite annual reward
Elite members receive one qualifying treatment each year up to a $350 value, from the list published on the memberships page.
Combining offers
Member discounts apply to regular-priced services and cannot be combined with promotions, specials or package pricing.
Credit value
Banked credit has no cash value, is transferable to another person, is nonrefundable, and may be used while your membership is active.

Changing or ending a membership

  1. After your commitment term, a membership continues month to month until you cancel. Cancel by contacting us in writing before your next billing date.
  2. During your commitment term, cancellation requires payment of the remaining months, unless we agree otherwise in writing.
  3. Failed payments. If a payment fails, we will attempt to reach you. A membership may be suspended, and banked credit unavailable, until the account is current.
  4. Upgrades and changes to a different membership tier take effect on your next billing date.
  5. We may change membership pricing or benefits with at least 30 days' notice. If you do not accept a change, you may cancel without penalty before it takes effect.

Article SixPromotions and gift certificates

Promotions published on our specials page or sent by email or text are subject to the terms stated with each offer. Unless the offer says otherwise:

  • Promotions apply to new bookings only and cannot be applied to a treatment already performed or already paid for.
  • Offers cannot be combined with each other, with membership discounts or with package pricing.
  • Offers have a stated end date and are not extended after it passes.
  • Offers are limited to one per person unless stated otherwise, and may be limited in quantity.
  • We may modify or end a promotion at any time. A promotion already redeemed is honored.

Third-party vouchers and daily-deal certificates are honored according to the terms printed on them and the law of the state where they were sold.

Our content

All text, photographs, graphics, logos, treatment guides and page designs on this website are owned by Cosmetiq Medicine LLC or used with permission, and are protected by copyright and trademark law. You may view and print pages for your personal, non-commercial use.

You may not copy, republish, scrape or redistribute our content for commercial use, remove any copyright or trademark notice, or use our name, logo or clinical photographs without our written permission.

What you submit

  1. Photographs you upload to our treatment planner are used to answer your question and prepare for a consultation. Use of the tool is optional, and you may describe your concern in text instead.
  2. You confirm that anything you submit is yours to submit, is accurate, and does not infringe anyone's rights.
  3. Clinical photographs taken during care are used to document treatment. We publish an image only with your separate written permission, which you may decline without affecting your care and may withdraw at any time.
  4. Reviews and comments you post publicly are your own. We do not edit reviews, and we do not offer anything of value in exchange for a positive review.

Acceptable use

Do not use this website to transmit malicious code, to attempt unauthorized access to any system, to scrape content in bulk, to impersonate another person, or for any unlawful purpose. We may suspend access to anyone who does.

Links to other sites

We link to websites we do not operate, including our booking platform, financing providers and the informational sources cited in our treatment guides. We are not responsible for their content, accuracy or practices, and a link is not an endorsement.

Article EightCalls, texts and email

By giving us your phone number and agreeing to be contacted, you consent to receive calls and text messages from us about appointments, aftercare and, where you have agreed to it, treatments and offers. Message frequency varies and standard message and data rates may apply.

Reply STOP to any text to opt out, or HELP for assistance. Consent to receive marketing messages is never a condition of purchasing any treatment. Marketing emails include an unsubscribe link. Opting out of marketing does not stop service messages such as appointment confirmations.

We do not share mobile numbers or text-message consent with third parties for their own marketing. How we handle your information generally is described in our privacy policy.

Article NineDisclaimers and limitation of liability

Website disclaimer

This website is provided on an “as is” and “as available” basis. To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose and non-infringement. We do not warrant that the website will be uninterrupted, error-free or free of harmful components, or that the information on it is complete or current.

Indemnification

You agree to indemnify and hold harmless Cosmetiq Medicine LLC and its owners, providers and staff from claims, damages and reasonable legal fees arising from your breach of these terms, your misuse of the website, or your violation of any law or the rights of a third party.

Article TenDisputes and governing law

Talk to us first

If something goes wrong, contact us before taking any other step. Most concerns are resolved quickly by a conversation with the clinic. Please give us 30 days from the date you notify us in writing to resolve a dispute informally before beginning a formal proceeding.

Governing law and venue

These terms are governed by the laws of the State of Oregon, without regard to its conflict of laws rules. Any action arising from these terms or your use of this website shall be brought in the state or federal courts located in Multnomah County, Oregon, and you consent to the jurisdiction of those courts. Nothing here limits any right you have under the consumer protection law of your home state.

General provisions

  1. Severability. If any provision is found unenforceable, the rest of these terms remain in effect and the unenforceable provision is limited to the minimum extent necessary.
  2. No waiver. If we do not enforce a provision, that is not a waiver of our right to enforce it later.
  3. Assignment. You may not assign these terms. We may assign them in connection with a merger, acquisition or sale of assets.
  4. Entire agreement. These terms, our privacy policy and any signed consent or membership agreement are the entire agreement between us on these subjects.

Changes to these terms

We may update these terms. When we do, we will post the revised version here and change the date at the top. Changes apply to bookings made after the change takes effect. If a change materially affects your rights, we will give notice before it takes effect, by email or a notice on the website. Continuing to use the website or book appointments after that means you accept the revised terms.
